What should I bring with me to each appointment?Please bring to each appointment a Driver's License and/or other picture ID, as well as your current insurance and prescription cards. You should have filled out our new patient paperwork in advance and returned it to our office prior to your first visit with is. CLICK HERE to download a copy of our new patient paperwork. For your safety, it is important that we reconcile your medication list on every visit. Therefore, we ask that you bring with you to each visit every medication and over-the-counter item (including vitamins or herbal supplements) that you take on a regular basis and whether at your own desire or prescribed by another physician or care provider. We understand this may be viewed as a nuisance, but it's important to understand that many vitamins and herbal supplements interfere with certain medications and may render them ineffective or harmful. If possible, you should have your medical records transferred before your initial visit. Be sure to bring any past records or test results that you have in your possession. Some may be copied for our chart and returned to you and others may simply be returned to you. It's helpful to write down questions you have so you can be sure to remember them for your visit. You should also bring something to take notes on. It should go without saying that just as you expect your physician to respect your time, you should respect your physician's time. This means you should refrain from accepting phone calls, reading emails or engaging in other activities that divide your attention when with your physician. We believe that the ideal patient-physician relationship is one of openness, respect and communication; and that it's this partnership that leads to the highest quality of care and the best possible outcomes for the patient. What other costs may be involved?For a list of other office charges CLICK HERE Do you see out-of-state patients?No, an internal medicine physician is a specialist in diseases affecting internal organs: a highly trained personal physician for patients, generally 18 years of age and older. Internists provide general medical care, advice to promote wellness and disease prevention and nonsurgical treatment of diseases affecting the internal organs. In other words they become the patients' primary care physician who establishes an individualized plan of care and follows the patients from then on throughout the end of life. How do I get copies of my medical records?You may request a copy of your medical records in part or in whole by completing IMG's Medical Records Request Form (CLICK HERE). Once you have completed the form you may fax it to our medical records department 904.346.0212 for processing. For additional information call 904.517.5704. Please note, we do charge a reproduction fee of $1.00 per page for the first 25 pages and .25 cents per page thereafter How long is an appointment?Appointment times vary based on individual need. However, generally a first appointment will take approximately one to one and a half hours. As a general rule, subsequent follow up visits are booked in 15, 30, 45 or 60 minute increments based on your physician's instruction. When should I schedule a follow-up appointment?Your physician will determine how often you need to be seen. Because of the busy nature of our practice, it's best to make your next follow-up appointment at the end of each appointment. Generally, Internal Medicine follow up appointments are booked every 3, 4, 6 or 12 months, based on your personal health condition. What do I need to do if I'm referred to another physician?Once your Physician places an order for you to be referred outside our practice to another physician or for other services, our referral staff will begin processing your referral. Most orders are processed within 7-14 business days based on urgency. Once the order/referral is processed, someone on our staff or from the other practice will contact you regarding your appointment. If your referral is needed urgently or ASAP it will be handled expeditiously. What is your cancellation policy?A $25.00 MISSED APPOINTMENT FEE WILL BE CHARGED FOR MISSED OR CANCELLED APPOINTMENTS WHEN 24 HOURS PRIOR NOTICE IS NOT GIVEN. What are your fees?In general, a new patient visit may run anywhere from $160.00- $300.00 NOT including lab tests, x-rays or additional procedures or services. It is important to note that your physician is responsible for ordering tests and procedures for you based on your health needs and the standard of care, not based on your particular insurance or even your ability to pay. How do I request a new prescription or a refill?New prescriptions can only be issued by a physician, therefore, contact your physician's clinical staff to determine if your physician will need to first see you or if a prescription can be issued without an appointment. CLICK HERE for a list of clinical staff numbers. Refills: Refills must be determined carefully, sometimes involving extensive review, and or prior authorization from your insurance company. Many medications require that you have blood work on a regular basis and that too must be determined prior to refilling your prescriptions. Simply have your pharmacy request a medication refill and we will determine what needs to be done in order to get it approved for you. Generally, refills can be approved within 3 days, however, there are times that authorizations can take up to two weeks. Or, you may request a refill online via our Secure Patient Portal on our website. (CLICK HERE) Can I pay my bill online?Yes, once you are registered through our Secure Patient Portal. (CLICK HERE) Do you bill insurance?Yes, we bill insurance for the many plans we are contracted with. However, insurance contract changes may result in participation status change. While we make every effort to notify our patients of major insurance plan changes, it is your responsibility to confirm with our staff each time you make an appointment that we are still participating with your insurance company. For additional insurance questions you may contact our insurance department at 904.854.9887 or through “Ask-A-Billing Question" online via our Secure Patient Portal (CLICK HERE). May I email my physician if I have medical questions between visits?No, you may not email physicians directly. However, once you have created your account through our Secure Patient Portal (CLICK HERE) you may email medical questions to your physician's clinical staff, by choosing Ask “Your Doctor's" Clinical Staff a Question. Emails will be exchanged between Physicians and Clinical Staff. At that point, we are able to answer many simple email questions. More complex medical questions/issues may only be addressed with an appointment. If addressing or treating your medical concerns results in saving you an office visit there may be a charge based on the amount of time and complexity involved in the decision making by the physician. Please keep in mind that our office can receive hundreds of emails daily, reading and responding takes considerable time for the physicians and staff. Please allow reasonable time for your email to be answered. What laboratory tests do you require prior to an appointment?Usually none prior to your appointment. This is because we take an individualized approach to treatment and can't know without gathering background on you during your appointment what tests will be necessary. Additionally, today insurance companies do not allow us to order routine lab test without a specific reason or diagnoses to warrant the tests. After meeting with the physician, certain laboratory investigations may be recommended. The charge for lab tests vary, and are not included in the consultation or visit costs. Some tests are covered by many insurance companies, some not. Your physician will assist you in determining which tests are absolutely necessary. How long before I receive results on labs and other tests?While some lab results are back within 3-5 days, many other lab test results may take 7-14 days or longer, report times will vary by Physician. If you have not heard from us after 14 days – please contact our office (CLICK HERE). Critical lab results are always reported to you immediately. Other test result times will vary by facility and Physician. Every test we order on you, either done on-site or off-site will be reported to you – We will either notify you by phone or via our Patient Message Retrieval system. Information provided to you on a green card upon check in at your office visit. if you have not heard from us within 14 days of having any test performed, please contact your Physician's clinical staff to request results.
